Continuum Care Strategies (CCS) is a rapidly growing, multidisciplinary care management company seeking experienced healthcare and human services professionals to join our exceptional team.

We provide comprehensive care coordination and advocacy for individuals and families navigating complex medical, cognitive, behavioral, and psychosocial challenges. Our care managers exercise independent clinical judgment while collaborating with experienced professionals across disciplines.

What You'll Do

• Conduct comprehensive assessments and develop individualized care plans.

• Independently manage complex cases and coordinate services across healthcare settings.

• Advocate for clients and collaborate with families, physicians, attorneys, and other professionals.

• Navigate challenging care decisions, crises, and transitions.

What We're Looking For

• Master's degree in a healthcare, clinical, or human services discipline.

• Minimum of five years of relevant professional experience.

• Strong clinical judgment, communication, and problem-solving skills.

• Experience in geriatrics, social work, nursing, behavioral health, rehabilitation, IDD, or complex case management.
