Case Manager and Certified Dementia Practitioner

Tyler Lloyd is a Care Manager with more than a decade of experience spanning healthcare, behavioral health, aging services, care management, and community-based support. His background includes work with older adults, individuals with serious mental illness and substance use disorders, people experiencing housing instability, and families navigating dementia, hospice, home care, and complex medical needs.

Before joining Continuum Care Strategies, Tyler served as a Medicare Case Manager with Johns Hopkins Medicine, where he managed a large panel of older adults with Medicare and Medicare Advantage coverage and worked closely with primary care physicians, community providers, and insurance teams to coordinate care and address medical, functional, and social needs. His experience also includes behavioral health care management, assertive community treatment, substance use services, hospice and palliative care, home care, and dementia care navigation.

Tyler has spent much of his career working across the places where healthcare can become difficult to navigate: hospitals, physician practices, skilled nursing facilities, assisted living communities, home health and home care agencies, behavioral health programs, insurers, and community organizations. That breadth gives him a practical understanding of how different parts of the care system operate and where communication, access, or follow-through can break down.

He is particularly skilled at building relationships, gathering information from multiple sources, explaining complicated services and benefits in understandable terms, and helping clients and families determine what needs to happen next. Tyler brings persistence, resourcefulness, and a strong understanding of the continuum of care to his work, with a focus on helping clients access appropriate services, navigate transitions, and maintain continuity when multiple providers and systems are involved.
